Welcome to Smile Quest Dental.  Please use the forms provided on this page to make your initial visit a great experience.  As your dental provider, we will need the following forms completed by you.  You can print them out using the menu to the left, or complete the forms in our office upon arrival.  The forms you will need are listed below:

2. HIPAA Consent Form
3. Cancellation Acknowledgment Policy 

To print each form, click on the respective link to the left, click on the document image to open it, and print the document.  Complete the documents and bring them to the office on your first visit.
